According to data released by The Joint Commission in April 2023, serious patient safety events in healthcare facilities rose by 19% in 2022, compared to 2021. The Joint Commission found that “failures in communication, teamwork, and consistently following policies” were primarily behind these incidents, the accreditation body reported.
